Who Needs Immunizations?
All children ages 0-5 years of age. School age children provide them to their public school.

Why are Immunizations Required?
Proof of immunization is a law required by the State of Utah in every school and childcare facility in the state.

How To Get Us Your Immunizations?
You can bring us a copy or email it to us at Jillian@LittleTigerPreschool.com, we do not have a fax machine.

Your doctor will know which immunizations are required or visit www.immunize-utah.org for more info.

Choosing Not to Immunize?
In order to be exempt from immunizations, you’ll need to do one of the following:

• Obtain a certificate from a licensed physician stating that due to the physical condition of the student one or more specified immunizations would endanger the child’s life or health
• A completed health form from the local health department where student lives, witnessed by local health officer
• A statement that the student is a bona fide member of a religious organization whose teachings are contrary to immunizations, signed by legal parent or guardian
• Beginning July 1, 2018, the Department of Health has an online option with an educational module parents can go through. Visit www.immunize-utah.org for more information.
